What is the Heart Care Partners Program?
When your resident enrolls in the Heart Care Partners program, they are getting help from a team of professionals for focused heart failure management. Customized care may stabilize or even improve symptoms.
We Offer
- Medications, equipment, and supplies to optimize heart function and minimize common heart failure symptoms.
- Information on diet, fluids and physical activity, plus consultation with a dietician and occupational therapist as appropriate.
- Regular check-ins to see how their care plan is working and make adjustments as required.
- An action plan and an emergency kit tailored to your community so we and your staff can manage a sudden change in symptoms, and avoid sending residents to the hospital or ER.
